New Patients

To enroll as a new patient with Ford Orthodontics:

1. First, contact our office by phone to schedule an appointment.
2. PLEASE CLICK HERE TO FILL OUT YOUR NEW PATIENT FORMS.
3. Once the forms are completed, click submit and your information will be merged into our computer system. You DO NOT need to print out your forms and bring them with you.
